How you can prepare for surgery

You'll want to keep your wait time to a minimum by completing the downloadable forms well before coming into the office. The day of the surgery, it is important to have the following on hand:
  • Your current glasses, sunglasses or contact lenses
  • A list of all medications you are taking
  • Your insurance card
  • A form of payment for your insurance co-pay
  • The name, address and phone number of your preferred pharmacy
